When completing the In Service of Yalahar questline, make sure to follow Yalahari on mission 9 (choosing a side-Evil - Also unlocks Follower of Azerus achievment).
Once you complete the Blood Brothers Quest, bring Palimuth your Vampiric Crest to unlock the final Evil-side outfit, which will unlock this achievement.
