In order to unlock this achievement, you must Complete the "A Retched Beginning" task from the Dream butterfly.
To do this, you must slay over 100 Retching Horror , but thankfully your counts before taking the task still count towards this achievement, so it isn't too hard.
You will also receive 3 Essence of Dread ] when handing in your task as well.
